Before we can begin our work together, you must complete the forms below. To make the best use of our time together, I encourage you to print and to complete them before your first appointment. If you cannot do so, please let me know, so that I can bring you a copy to complete before we start.

FEES AND PAYMENT

Sessions are 53 minutes in length at a rate of $180 per session. Therapy for couples or families has a rate of $200 for a 53 minute session. Clients who need or desire a session of 75 minutes or of 100 minutes may schedule those, and the rate will be adjusted accordingly. Some sliding scale slots are available for uninsured individuals/families who request them.

Currently, I am able to accept most forms of Blue Cross/Blue Shield, including the NC State Health Plan. If I am in network with your plan, then you only owe your co-pay amount at the time of our sessions. If you want to seek reimbursement from a different insurance company, I can provide you with weekly statements, but you are responsible for paying the full amount. I cannot guarantee how much, if any, your company will reimburse. Failure to show for an appointment without at least 24 hours notice will require full payment by the client; insurance will not contribute to the cost of missed appointments.

Payment is due at the time of service. I prefer Venmo payment, but can accept cash or checks. Hope Haven’s Venmo account is set to private, so no one can see your transaction. Please see the Venmo Agreement in the next column for details. None of your health information is connected to your Venmo transaction. I cannot accept any form of credit card or debit card.

LOCATION/CONTACT

Hope Haven Therapy is located within the Fayetteville Road Office Park II complex, near Highway 54 and Hope Valley Rd. From 54. The address is 6208 Fayetteville Rd., Suite 106, Durham, NC 27713. The office is conveniently located close to the I40 exit and Southpoint Mall. (Clicking on the map below will open up directions in Google Maps.) Clients participating in teletherapy will receive a unique link for their sessions.
